Top prizes are awarded to the students and adults who raise the most funds. There are 3 Top Student prizes, and 3 Top Adult prizes. Families can fundraise individually or collectively.

Waiver *

I have read and certify my agreement to the below Release of Liability Waiver

I hereby understand and assume full and complete responsibility for any injury or accident, which may occur during my participation in Walk for Life or while I am on the premises of the event. I hereby release and hold harmless Advokate Life & Education Services Society, the sponsors, promoters, and all other persons and entities associated with the event or their agents or employees, or otherwise. I will not enter and participate unless medically and properly prepared. I assume the risk associated with this event, including but not limited to falls, contact with participants, the effects of weather including high heat and/or humidity, and the conditions of the trail, all such risks being known and appreciated by me. I further agree to assume full responsibility for all minors I bring with me under my care and I hereby release and hold harmless Advokate Life & Education Services Society for any and all risk associated with this Walk for Life event for the minors under my care.

Media Release *

By participating in the Walk for Life event, I understand and acknowledge that my face and/or my likeness may appear in various photographs and videos taken at the event and I hereby grant Advokate Life & Education Services Society my permission to use any photographs, videos, motion pictures, recordings, or any other record of myself and my family of this event.
